This Privacy Policy explains how SolisMed Home Health Care
Inc collects and uses information about you in relation to its text message service (the “Messaging Service”). 
By using the Messaging Service, you agree to the terms of this Policy. SolisMed
Home Health Care Inc reserves the right, in our sole discretion, to modify or change this Policy at any time, with or without prior notice to you.

Collection of Information

We collect various information on our behalf from and about you, including information you directly provide when you use the Messaging Service. For example, we collect the phone number you provided when signing up for the Messaging Service. When you send messages via the Messaging Service, we will also collect your messaging history and any information included in those messages. We may also collect personal and medical information including but not limited to patient and family member names, gender, age, addresses, medical information, medication information, allergy history through our HIPAA compliant third party provider.

Use of Information

We use your information to establish the medical records for patients, database for contact information system with patients and family members, generate services, and analyze and improve our services to patients and family members. We may also use your information to enhance the Messaging Service features and customize and personalize your experiences on the Messaging Service. 


Sharing of Information


SolisMed Home Health Care Inc will not share or sell your Personal Information to other companies or individuals. No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties. We may disclose your information if you consent to us doing so, as well as in the following circumstances: Medical Service Providers. We may disclose your information with third parties, such as Medicare, Medicaid, Medi-Cal, hospitals, physician clinics, emergency departments, 911, nursing homes, when it is necessary for further medical services from these third parties.


Protection of Information


We take a variety of physical, technical, administrative, and organizational security measures based on the sensitivity of the information we collect to protect your information against accidental or unlawful destruction or accidental loss, alteration, unauthorized disclosure or access. Unfortunately, no online activity can be guaranteed to be 100% secure. While we strive to protect your information against unauthorized use or disclosure, we cannot ensure or warrant the security of any information you provide. By using the Messaging Service, you agree that SolisMed Home Health Care Inc is not liable for any unintentional disclosure.

Children and Intended Audience

The Messaging Service is not intended for children under 13, and SolisMed Home Health Care Inc does not knowingly collect information from children under the age of 13.

Children aged 13 or older should not submit any Personal Information without the permission of their parents or guardians. By using the Messaging Service, you are representing that you are at least 18, or that you are at least 13 years old and have your parents’ permission to use the service.


Retention of Information

We retain your information for as long as you participate in the Messaging Service or as needed to accept our medical services and comply with applicable legal obligations. We will also retain and use your information as necessary to resolve disputes, protect us and our customers, and enforce our agreements.

 

Opting in

​

A mobile user might opt-in by: Entering a phone number online; Sending a message with “START”, “SUBSCRIBE”, “AGREE” to our message service. No form is needed to be filled.

 

Message frequency

​

The message frequency may vary depending on the patient’s status and need of the medical services. We will not send messages to you without proper reason or need. We only send messages when it is needed, for example, the notice and reminder of service, quality assurance survey and feedback, necessary information request.

 

Opting out

​

You can opt out by texting “STOP”, “UNSUBSCRIBE”. After opting out, you can opt in again any time by “START”, “SUBSCRIBE”, “AGREE” to our message service. However, you are aware and agree that if you opt out of our message system, it will be your responsibility that medical service may be delayed or even canceled if we are not able to contact you or send you information regarding the services and health notices.
